Payment: Payment is due at the time of booking for all acupuncture services provided by COLE ALEXANDER DAOM, PLLC. We accept payments through credit cards and checks. For your convenience and safety, please note that we do not keep cash on the premises. If you prefer to pay in cash, kindly bring exact change and notify us in advance.
Insurance and Superbills: COLE ALEXANDER DAOM, PLLC does not directly handle insurance billing. However, as required by law, we are happy to provide a superbill for your acupuncture treatment. Please note that additional administrative tasks, such as changing ICD diagnosis codes, rewriting notes, submitting extra documentation, performing additional diagnostic tests, testifying in court, or completing supplementary paperwork, are not included in our services.
Cancellation and Rescheduling: We understand that unforeseen circumstances may arise. If you need to cancel or reschedule your appointment, kindly provide at least 24 hours’ notice. This allows us to accommodate other clients who may be waiting for an appointment. Appointments canceled with less than 24 hours’ notice may be subject to a cancellation fee.
Late Arrivals: We strive to provide every client with a full and undisturbed session. If you arrive late, your session may need to be shortened to accommodate the next scheduled appointment. The full session fee will still apply.
No-Show Policy: If you miss an appointment without providing any prior notice, it will be considered a no-show. No-shows may be subject to a fee equivalent to the full cost of the scheduled appointment.
Refunds: Refunds are generally not provided for completed sessions. If you have concerns or are dissatisfied with your treatment, please communicate with Dr. Alexander during your session, and we will do our best to address your needs.
In-Home Services: For in-home services, please ensure a safe and comfortable space for treatment. In the event of unsuitable conditions or discomfort, Dr. Alexander reserves the right to terminate the session, and the full fee will still apply.
