Thank you for choosing COLE ALEXANDER DAOM, PLLC. We look forward to helping you to meet your healthcare goals. Please carefully read the policies below:
FINANCIAL RESPONSIBILITY & PAYMENT TYPES
COLE ALEXANDER DAOM, PLLC is committed to providing transparent billing practices. Your financial responsibility and the timing of your payment depend on your specific coverage type:
- Self-Pay: Payment in full is due at the time of booking. When we schedule your first visit with you by phone, payment for that visit is due when you arrive. For your convenience, we accept credit cards, and cash on request.
- Health Insurance:
- In-Network: Any required co-pays, co-insurance, or deductibles are due at the time of check-in, prior to the commencement of your treatment. You are financially responsible for the full balance if your insurance company denies a claim, retroactively closes coverage, or determines a service is not medically necessary.
- Out-of-Network: If we are not credentialed with your insurance carrier, you will be treated as a Self-Pay patient with payment due at booking. Upon request, we will provide a “Superbill” detailing the services provided and charges incurred. You may submit this Superbill to your insurance company for potential reimbursement. The Superbill does not include additional administrative work, such as submitting extra documentation, performing additional diagnostic tests, testifying in court, or completing supplementary paperwork. Diagnosis codes and chart notes record what was found and done at your visit. We correct a genuine error when you ask in writing, but we do not change codes or notes to meet an insurance company’s requirements. If you plan to submit a Superbill to your insurance, please obtain an approved ICD-10 diagnosis code from your primary care provider, another referring provider, or your insurance company, and give it to us before treatment begins. It is your responsibility to understand the terms of your insurance coverage. COLE ALEXANDER DAOM, PLLC and its providers are not responsible for any amount not reimbursed by your insurance.
- WA State Labor and Industries (L&I): We strictly adhere to all WA State L&I procedures. Before commencing or continuing care, it is our responsibility to verify an active claim, an open and allowed condition that covers acupuncture, and any required prior authorizations. You will not be billed out-of-pocket for any treatments properly covered under your allowed L&I claim.
CANCELLATION & NO-SHOW POLICY
COLE ALEXANDER DAOM, PLLC provides healthcare services by pre-booked appointments only. Proper planning is crucial to reliably schedule visits and ensure all patients have access to care. A full 24-hour notice is strictly required for all cancellations and rescheduled appointments. Consequences for late cancellations (less than 24 hours notice) and no-shows are based on your payment type:
- Self-Pay and Health Insurance Patients: You are personally responsible for paying missed appointment or late cancellation fees, as insurance will not cover these charges. Missed or late-canceled appointments will not be refunded. Patients who miss or late-cancel more than two (2) appointments within a one-year period will lose the privilege of booking appointments in advance. Future visits will be strictly restricted to walk-in or day-of scheduling, subject to availability.
- WA State Labor and Industries (L&I) Patients: Missed appointment fees cannot be billed to L&I or the patient. However, all no-shows and late cancellations will be documented and formally reported to your Claim Manager, which may negatively impact your claim. Repeated non-compliance with your treatment schedule may result in discharge from our clinic and a referral back to your Attending Provider.
Late Arrivals: We strive to provide every client with a full and undisturbed session. If you arrive late, your session may need to be shortened to accommodate the next scheduled appointment. The full session fee will still apply. For L&I patients, the visit is billed to L&I and you pay nothing.
Refunds: Refunds are not provided for completed sessions, except that any amount you have paid beyond what you owe will be refunded. If you have concerns or are dissatisfied with your treatment, please communicate with your provider during your session, and we will do our best to address your needs.
In-Home Services: For in-home services, please ensure a safe and comfortable space for treatment. In the event of unsuitable conditions or discomfort, your provider reserves the right to terminate the session, and the full fee will still apply.
Returned Check Fee: A $40.00 fee will be assessed for any checks returned by your bank due to insufficient funds or closed accounts. This fee must be paid in full before any future appointments can be scheduled or attended.
If your health plan’s rules or the law do not allow us to charge you for something described in this policy, we will not charge you for it.
If you are paying for your care yourself, you have the right to a Good Faith Estimate of your expected charges. Learn more on our Good Faith Estimate page.
